How to Submit Your Completed Forms
Once your form is complete, you can fax it to our office at (425) 455-9946 or bring the printed copy with you to your appointment. If you have previous medical or vaccination records from another veterinarian, please include those as well, or ask your former clinic to send them to us before your visit.

Should I bring my pet’s previous medical records?
Yes. Please bring vaccination and medical records from your previous veterinarian, or ask them to send the records to us before your visit.
What if I can’t print the form at home?
No problem. Just arrive a few minutes early to your appointment and our front desk team will help you complete the form in our office.
